Referral Process & Documentation
If you would like to make a referral to the service please follow the appropriate referral path:-
For schools already within the Partnership
- Please liaise with the nominated member of staff within school who can identify whether a referral is appropriate and can authorise the referral.
- Please discuss the referral with parents, carers and any interested parties such as social care as referrals cannot be progressed without consent of the adults who hold parental responsibility. Please complete the referral form providing as much detail as is possible.
- Please obtain the signed consent of the parent/carer on the rear of the referral form and also complete the consent form.
- Please password protect and email referral forms to the following email address:-thebungalowpartnership@gmail.com
- The Bungalow Partnership will then confirm receipt of your referral and assess the support needs of the child/young person.
For schools not within the partnership
- Please contact The Bungalow Partnership to discuss the child/young person’s needs.
- If following a consultation it is agreed that a referral to The Bungalow Partnership is appropriate a referral form will be issued.
- Please complete the referral form, gaining signed consent on the rear of the referral form and also complete the consent form with the person with parental responsibility.
- Please email the completed referral (password protected) to:- thebungalowpartnership@gmail.com
- We will confirm receipt of the referral and allocate to an appropriate worker and invoice the school accordingly.
For private referrers (Parents and Grandparents)
- Please contact the Bungalow Partnership to discuss the needs of your child.
- If following a consultation it is agreed that a referral to The Bungalow Partnership is appropriate a referral form will be issued.
- Please complete the referral form and sign to authorise that you consent to the therapeutic support. We also need a signed consent form.
- Please return the signed referral and consent form by post or email (password protected if by email) to:- thebungalowpartnership@gmail.com
- We will then confirm receipt of your referral and contact you to advise when the support will begin.
For Local Authorities
Please follow the same route as for Schools not within the Partnership and gain funding authorisation from relevant Team Manager.
